Primary Care Associates - Location List
Primary Care Associates has five convenient locations in Portage and Summit Counties. Our practice offers on site testing including nuclear stress testing, echocardiogram, cartoid doppler, vascular testing and MRI and CT scans. We also offer physical therapy services at our Kent and Stow locations. Our practice also provides personalized hospital care to our patients.
Office Policies
There is a $25 charge for not showing up for an appointment, or for any appointments that are cancelled without 24-hour notice.
All copays are due at the time service. If your copay is not paid at the time of service, you will be charged a service fee of $20.
All prescription refills are to be done at your office visit. Prescriptions will not be called in to your pharmacy if you have been recently seen by your physician.
Your insurance card must be presented at every office visit.
